Se realiza una incisión en el pliegue superior del párpado y otra en la parte inferior debajo de las pestañas o en el interior del párpado. A partir de estas incisiones, se separará la piel del tejido subyacente y se retira el exceso de grasa y piel flácida. La incisión se cerrará con pequeñas suturas, que se retirarán dentro de cuatro a cinco días después de la cirugía.
Los pacientes que tienen solamente exceso de grasa en los párpados de abajo (no exceso de piel), son candidatos a la cirugía por dentro del parpado. Evitando una incisión externa sin embargo, requiere de sedación intravenosa para realizarla.
